One dead, another injured in SH-46 head-on crash near Boerne

Levi Timothy James, 22, died after being taken for treatment following the collision near Boerne. DPS is seeking witnesses as investigators examine why his car entered opposing traffic.

One driver died and another was injured in a head-on collision on State Highway 46 near Boerne on Wednesday, November 4. Levi Timothy James, 22, of Pipe Creek, died after being taken to a medical facility. The other driver’s condition remains unknown in the published accounts of the crash.12

The Texas Department of Public Safety said James was driving east in a Mazda when his car crossed into opposing traffic and collided with a Mitsubishi around noon in Kendall County. Investigators are still trying to establish why the Mazda entered the other lane—a question the reported collision sequence does not answer.12

Key facts

  1. 1Driver died after transport for treatment 1
  2. 1Other driver transported with injuries 1

The San Antonio Express-News identified the vehicles as a Mazda Miata and a Mitsubishi Outlander SUV, attributing those details to DPS Sgt. Dila Hidalgo. James lived in Pipe Creek, an unincorporated community in neighboring Bandera County. The collision occurred near Boerne, not in his home community.2

James was not reported to have died at the scene. KSAT’s account specifies that he was transported to a medical facility and later died there. Neither local report identifies the facility or gives a time of death, so the approximate noon crash time should not be read as the time he died.12

What the lane crossing establishes

Hidalgo told the Express-News that James’s car “crossed into the oncoming lane of traffic and collided with the Mitsubishi Outlander.” That is DPS’s account of the movement immediately preceding the impact. The same report says officials were investigating why the car crossed over; it does not give a completed investigative finding.2

That distinction matters. Describing which vehicle entered opposing traffic is not the same as explaining what caused it to do so, or reaching a legal determination of fault. The available reporting establishes the direction James was traveling and the head-on collision, but it does not establish an underlying cause.

The two local accounts are consistent on the central sequence and the fatal outcome, but offer different levels of detail. The Express-News names the vehicle models and the DPS spokesperson. KSAT makes clear that James died after transport and that the Mitsubishi driver was transported with injuries. Both accounts attribute their information to DPS.12

The reported time is approximate. The Express-News describes the crash as occurring at noon Wednesday, while KSAT says around 12 p.m. Those descriptions support a midday collision on November 4, 2026, not an exact minute. There is no substantive disagreement between the accounts on that point.12

The injured driver and witness appeal

DPS did not identify the Mitsubishi driver in the Express-News report. That account said the driver was taken to a hospital but provided no injury details. KSAT’s later account explicitly reported injuries, without describing their severity. Neither account establishes the driver’s subsequent condition or prognosis.12

Hospital transport therefore remains the last reported point in that driver’s medical care. It would be wrong to turn the absence of further information into an assurance of recovery—or an assumption that the injuries were life-threatening. What is confirmed is that the driver was injured and taken for treatment.1

DPS is asking anyone who witnessed the collision to contact the department at 210-531-2284, according to KSAT. That is the published witness-information number for this crash. For someone who saw the vehicles before or during the collision, the department’s appeal provides a direct way to pass that information to investigators.1

The next meaningful developments would be a DPS finding explaining the lane crossing and a verified update on the injured driver. Until then, the account remains limited but consequential: James died after transport, another driver was hurt, and the reason the two vehicles came into each other’s path remains under investigation.12
