Largest US Cyclospora Outbreak Ever: 47 States Ill, Michigan Tops 10,000 Cases

Nearly 23,000 people were ill across 47 states in early August, making this the largest recorded Cyclospora outbreak in the United States by a wide margin. The scale alone changes what the illness means: cyclosporiasis is usually discussed as a summer cluster tied to a restaurant meal or a single imported shipment, not as a national event that fills county health departments and forces a state to report deaths. By Aug. 5, the Centers for Disease Control and Prevention's weekly update put the outbreak at 47 states with nearly 23,000 cases ^{6 9}.

The first known illness onset was reported in early May 2026, with the CDC counting laboratory-confirmed cases since May 1 ^{9 5}. On Aug. 4, 2026, the CDC reported 10,468 laboratory-confirmed cases since May 1, an increase of more than 3,700 since its July 28 report ⁵. The gap between that laboratory-confirmed tally and the nearly 23,000 cases cited in the CDC's weekly update the next day reflects how different agencies count this parasite. Michigan's outbreak now appears to be waning, at least by the measures state officials trust most. Natasha Bagdasarian, MD, MPH, chief medical executive for Michigan, said the worst is behind the state based on the data available that day ⁸. Michigan residents were told they could return to eating lettuce and salad greens as cases decline ¹⁰. State officials also told residents they could return to their routine, a signal that avoidance advice was no longer seen as helping in Michigan even as the national investigation continued ⁸.

That guidance rests in part on biology and time. Experts believe that because lettuce is perishable, the contaminated produce was eaten or thrown away long ago ¹⁰. The phrase captures both relief and frustration: relief because the exposure window for the spring lettuce may have closed on its own, frustration because a product with a shelf life measured in days leaves investigators chasing records after the food itself is gone. The honest reading of this outbreak is that size did not bring clarity. Tens of thousands of illnesses would normally be expected to sharpen traceability, with more receipts, more interviews and more chances to converge on a meal. Michigan can plausibly tell its residents that lettuce is back on the menu because its curve has bent ^{10 8}.
