  1. On October 1, we reported Pennsylvania's fifth measles-associated death. Now state officials are investigating a sixth possible death.
  2. The outbreak reaches 43 counties, and Pennsylvania had already reported more than 200 hospitalizations by early October. When will promised federal investigative help begin, and can officials resolve the conflicting death counts?
  3. Lancaster County has appeared to be the outbreak's center. Franklin County is where officials are investigating the latest possible measles-associated death. Neighboring New York has declared a statewide emergency to expand vaccination and testing capacity.
  4. Pennsylvania recorded 1,260 cases across 43 counties in its October 16 update. That's 38 more cases and one more county than two days earlier, when the state listed 1,222 cases in 42 counties.
  5. Nationally, confirmed cases reached 4,410 through October 15. The previous weekly total was 4,080. That's another 330 cases, an increase of about eight percent in a week, with more than two months left in the year.
  6. The national hospitalization tally stands at a reported 451 for the year. These are cumulative hospitalizations, rather than admissions on a single day. The CDC identifies 47 reporting jurisdictions, a category that includes more than states alone.
  7. Pennsylvania counted 943 cases on September 30 and 1,004 on October 5. The total reached 1,078 on October 8, then 1,136 through October 9. It rose to 1,222 on October 14 and 1,260 on October 16.
  8. The CDC announced additional assistance on October 13, after Pennsylvania formally requested help through Epi-Aid, its emergency outbreak-assistance program. The proposal includes one scientist, often called a disease detective, to help investigate how the outbreak is spreading.
  9. Pennsylvania's request was followed by a plan to send the investigator later that week. But on October 15, the state health department said it hadn't made a final decision on accepting the assistance. Deployment arrangements were still unsettled.
  10. Federal officials have also reportedly pledged nearly four million dollars. Whether any of that money has been paid to Pennsylvania isn't clear. The pledge comes alongside the proposed investigative assistance.
  11. Six people diagnosed with measles have died since August. State officials distinguish five measles-associated deaths from a sixth possible death they're investigating. Counting everyone who died after a diagnosis leaves the sixth person's cause of death unresolved.
  12. The latest investigation concerns a 39-year-old unvaccinated woman who had underlying health conditions. Franklin County's coroner described the primary cause as “a probable acute cardiac event due to possible complications of measles.” The cardiac event was described as probable, and measles complications as possible.
  13. Pennsylvania's health department opened an investigation and was still investigating in its October 14 notice. The woman is a different person from the 40-year-old unvaccinated woman whose death was Pennsylvania's third reported death in September.
  14. The fifth death involved an unvaccinated Lancaster County resident who tested positive for measles. Two infants too young for vaccination were among the five deaths reported in early October. The newer investigation hasn't supplied a final cause-of-death finding.
  15. Governor Josh Shapiro and federal health secretary Robert F. Kennedy Jr. have disputed the response and death counting. A federal count cited on October 13 listed two deaths nationwide, without naming the states. That older figure still hasn't been reconciled with Pennsylvania's tally.
  16. Penn State Health physician Evan Shirey described severe brain and respiratory illness among children he'd treated. He blamed measles' return on failure to vaccinate. Pennsylvania officials described most hospitalized patients as unvaccinated.
  17. Nationally, most cases have involved people who were unvaccinated or whose vaccination status was unknown. Those groups need to stay separate. Someone with an unknown vaccination history may have received doses that haven't been documented.
  18. Pennsylvania has held pop-up vaccination clinics, with nearly 9,000 response doses administered by October 13. Health officials said the outbreak was largely affecting Amish communities with low vaccination levels. Cases extend beyond any single community.
  19. South Carolina's earlier outbreak reached nearly 1,000 cases. More than 15,000 MMR doses were administered in the affected area, and officials described bringing that outbreak under control in about four months. Those figures don't set a timetable for Pennsylvania.
  20. Governor Kathy Hochul's emergency order expands New York's vaccination workforce. It includes paramedics, advanced emergency medical services providers, midwives and pharmacists. The order also gives nurses greater flexibility to order testing, widening the state's capacity to find infections.
  21. The statewide authority runs from October 5 through November 4. New York also announced plans to seek federal assistance, including help from the CDC and the Department of Health and Human Services. Pennsylvania had already made its formal Epi-Aid request.
  22. New York counted 108 cases through October 3, including 101 outside New York City. Health commissioner James McDonald cited 124 in an interview aired October 6. He said two-thirds of the cases involved adults.
  23. October 13 reporting put New York at 126 cases, but didn't give an exact cutoff. That leaves the state's current total unclear. The emergency targets access to vaccination and testing, including in underimmunized rural communities.
  24. Kindergarten MMR coverage fell from 95.2 percent before the pandemic to a cited 92.4 percent, down 2.8 percentage points. The community protection benchmark is about 95 percent. A national average can hide much lower coverage in individual communities.
  25. About 280,000 kindergartners attended school in the preceding year without documentation that they'd completed MMR vaccination. That leaves uncertainty about completion, including among children who may already have received one dose.
  26. Two MMR doses are about 97 percent effective against measles. Routine doses are given around ages one and five. During outbreaks, some babies as young as six months can receive an early dose, followed by both routine doses.
  27. Measles spreads through the air when an infected person breathes, coughs or sneezes. Contaminated air can remain infectious for up to two hours. Symptoms commonly appear seven to fourteen days after exposure, and can take as long as twenty-one days.
  28. The United States eliminated measles in 2000, but outbreaks can still occur when the virus reaches susceptible people. The delay between exposure and symptoms gives infections time to spread before people recognize that they're ill.
  29. Federal investigative assistance is promised while Pennsylvania's case count continues climbing. Officials still haven't confirmed when the investigator starts work, or explained how the state and federal death counts fit together.
