- The Abuja patient denies visiting Congo, contradicting the itinerary Ugandan officials gave. Nigerian authorities identified 44 contacts before her Ebola diagnosis, including healthcare workers. Investigators still have to determine where she was infected and whether anyone else caught it.
- On October 21, we reported Nigeria's laboratory-confirmed case and 44 identified contacts. The new development is a direct conflict over her travel. Kenya's nine-case total was already known by October 20.
- Nigeria confirmed the woman's infection with Bundibugyo, the type of Ebola involved in this outbreak, on October 20. At that announcement, she was in isolation and receiving care after several days at a private hospital. Officials haven't given a later update on her condition.
- Abuja is the woman's home. Kampala is where she says she spent her holiday. Congo is where Ugandan officials say she travelled on business. Kenya has a separate cluster whose first patient travelled from Congo through Uganda.
- The woman is a Ugandan national living in Abuja with her husband, a Swiss diplomat. Ugandan health officials described a business trip to eastern Congo, followed by transit through Uganda on the way home.
- Africa CDC director-general Jean Kaseya says she told investigators she'd never visited Congo and had spent two weeks holidaying in Kampala. The disagreement concerns whether the Congo trip happened at all. Her place of infection is still unknown.
- Uganda's health ministry also cautioned that travelling through Uganda doesn't show that she caught Ebola there. And the arrival date we previously gave, October 5, is now disputed.
- The WHO's Africa office placed symptom onset on October 7, four days after her return. That implies October 3. A Nigerian disease-control official gave October 5 as her arrival date. Both chronologies put symptoms on October 7.
- She sought care for malaria-like symptoms on October 11. Doctors didn't suspect Ebola for more than a week. Laboratory confirmation followed on October 20, nine days after that hospital encounter. That delay widened the period investigators need to examine for possible exposure.
- Nigeria's health minister announced 44 identified contacts. Later descriptions say at least 44, including many healthcare workers. Authorities planned isolation for at least three weeks while watching for illness. Those people were identified for monitoring because of possible exposure.
- Officials haven't announced whether any contacts developed confirmed infections or whether everyone has finished monitoring. Each person's monitoring period depends on their own exposure date. Abuja's international travel connections add to the tracing challenge.
- Investigators need to reconstruct her movements through travel, daily activities and healthcare before diagnosis. A contact might have encountered her in any of those settings. Identifying that encounter helps determine who needs follow-up.
- Kenya shows how an imported infection can lead to a local cluster. Its first patient had lived in Congo, became ill there and received care there before travelling through Uganda to Kenya. The patient was diagnosed on October 5 and died that night.
- Kenya had three confirmed cases by October 16. Four more on October 17 brought the total to seven. An eighth followed on October 18, then a ninth on October 20. The later headlines didn't mark a new Kenyan foothold.
- The second and third patients were close family members. The third was found in quarantine. The four cases added on October 17 were three healthcare workers who treated the first patient and a close male relative. All four had reportedly been quarantined before testing positive.
- Those four were the first known infections acquired within Kenya. The eight additional patients are described as contacts of the first patient. On October 22, surveillance teams described collecting 14 samples from suspected cases, without announcing the results.
- That sampling doesn't add 14 confirmed infections to Kenya's nine. The cluster has travel and contact links to Congo, but investigators haven't shown a transmission chain connecting it to the Abuja woman.
- Uganda's earlier outbreak had 20 cases and two deaths before being declared over in August. The travellers' routes have prompted concern about missed infections. Their passage through Uganda doesn't demonstrate a renewed outbreak there.
- Congo had 9,552 confirmed cases and 4,602 deaths through October 18. A later tally gives 9,637 confirmed cases and 4,646 deaths by October 19. Those are dated snapshots, rather than totals for October 23.
- The outbreak began in Congo in May. By October 21, Africa CDC described it as spanning Central, East and West Africa. Within Congo, eight provinces were affected. Our October 20 coverage described the expansion into Nord-Ubangi, with two reported confirmed cases and one death.
- About 70% of new Congolese cases reportedly occur in the community. That suggests infections may be escaping early detection. It also makes finding sick people and their contacts essential to stopping further spread.
- Young children face a particular protection gap. Children under five reportedly account for nearly one in four cases and one in three deaths. Some vaccine trials exclude that age group. Bundibugyo vaccine trials are underway, but doses aren't expected for many more months.
- WHO director-general Tedros Adhanom Ghebreyesus says one imported case can be contained by quickly tracing contacts. He warns that the real danger is sustained spread within communities. Nigeria contained an imported Ebola cluster in 2014, drawing partly on its polio-response infrastructure.
- That experience gives Nigeria capacity to respond, while the present investigation still has to resolve the woman's exposure and the outcomes among her contacts. The competing itineraries and return dates remain unresolved.
- Kaseya warned that the whole continent could be affected if health authorities can't contain the outbreak. Africa CDC announced on October 21 that African health ministers will meet virtually on Wednesday, October 28.
