  1. A Qantas crew stopped its jet as a Virgin aircraft crossed in front.
  2. A missed radio safeguard preceded the taxiway conflict. The controller was alone in the tower, with a roster that predicted a high fatigue risk. Investigators still have to determine why the flight crew had to prevent a more serious outcome.
  3. The conflict happened on August 17, 2026. Investigators released their preliminary report on October 26. The new development is the investigation's release, more than two months after the aircraft crossed paths on the ground.
  4. Virgin flight VA1589 had diverted from Newcastle because of bad weather and was taxiing towards the domestic terminal. Both aircraft were Boeing 737s. Virgin's registration was VH-IXJ, and Qantas's was VH-VZV. The conflict developed at about nine forty in the morning, after both jets had landed.
  5. Virgin had landed on runway 16L and was travelling north on taxiway B. Qantas had landed on runway 16R and was leaving through taxiway B8. Their routes brought them towards the taxiway intersection where Qantas expected Virgin to give way.
  6. The controller gave Virgin two requirements in one transmission. Follow Qantas as it left the runway, then stop before runway 25. Pilots repeat clearances so the controller can check that they've understood them. In this exchange, the captain's reply included only one of those requirements.
  7. The controller said, “follow the Qantas to vacate ahead, hold short runway 25”. The captain repeated the hold-short instruction. The missing part was the instruction to follow Qantas. The controller didn't catch the incomplete reply and moved on to other tasks.
  8. The Qantas flight QF420 crew saw Virgin approaching at a speed that suggested it wouldn't give way. They stopped using normal braking, and Virgin passed in front. The Virgin crew didn't realise there had been a conflict until the Qantas crew queried it with the controller.
  9. The controller later told investigators that equipment inside the tower cab limited their view of the intersection. After the occurrence, the controller was removed from duty for the rest of the day. That was the immediate operational response.
  10. The radio exchange and the restricted view involved different safety barriers. The readback check didn't expose the missing instruction. Visual monitoring didn't prevent the conflicting movements. The investigation must assess how each circumstance affected what happened. The controller's roster brings a separate question about fatigue risk into that assessment.
  11. The scheduled interval between shifts was eight hours and forty minutes. The roster-based prediction put fatigue risk at high. The controller described their state as “fine and alert” and said they were “fit for duty”. Their actual fatigue remains unresolved.
  12. The roster had the controller finishing at eight fifty the previous evening and starting again at five thirty the next morning. That leaves eight hours and forty minutes between scheduled shifts, with travel and other activities taking time too. It doesn't tell us how much the controller slept.
  13. Airservices Australia, which provides air traffic control at Sydney, lacked a fatigue-management system approved by the Civil Aviation Safety Authority. Airservices acknowledged the interim reports and said it would review the findings once the investigations were complete.
  14. Investigators released a second preliminary report concerning an August 25 pushback conflict between Jetstar and QantasLink aircraft. Flight crews and ground staff stopped those movements. Together, the two releases form part of six ongoing close-call investigations at the airport.
  15. Chief Commissioner Angus Mitchell credited crews and ground staff with identifying the conflicts and acting. Investigators are examining whether staffing contributed to any of the six incidents, or whether Airservices was effectively managing it. That question hasn't been settled.
  16. The August 17 preliminary report contains no analysis, findings or recommendations. Qantas's recognition and stop protected the aircraft while the Virgin crew remained unaware of the conflict. Investigators still have to weigh the missed readback, restricted sightline and roster risk before deciding what changes are needed. A publication date for the final report hasn't been announced.
