- Ebola has killed a second person in Kenya, leaving seven patients in treatment.
- On October 21, we reported nine confirmed cases, including one death. The total remains nine, but two patients have now died. The woman who died was already included in that total.
- The health ministry says all infections still belong to one known cluster, with no evidence of community spread. The unresolved issue is whether everyone still being monitored can complete quarantine safely.
- The woman received care in Nairobi, and the seven remaining patients are being treated at two isolation facilities. Kenya is managing its first recorded Ebola outbreak, caused by the Bundibugyo virus.
- A single imported infection links the nine confirmed cases. The first patient was a 40-year-old Kenyan man who had been living in the Democratic Republic of the Congo. He travelled through Uganda and arrived in Kenya on October 3.
- A relative drove him to The Nairobi Hospital. He tested positive on October 5 and died that day. Two national laboratories confirmed the finding. Kenya announced the case on October 6, when a safe burial also took place.
- The second person to die was a 65-year-old woman, a close relative and direct contact of that first patient. She developed symptoms in quarantine on October 13 and entered an infectious disease unit the same day.
- Her infection was announced on October 16 as Kenya's third confirmed case. She received supportive care for ten days before dying early on the evening of October 23. Her family has been informed, and authorities have asked for privacy.
- The ministry is arranging a safe and dignified burial with her relatives. Trained teams are to respect the family's wishes and beliefs while protecting mourners from infection. Contact with the body of someone who died from Ebola can transmit the virus.
- The death followed a reassuring clinical update. In the 24 hours to October 22, the ministry described all eight patients then in treatment as stable and responding well. One subsequently died, so that earlier description can't describe everyone's condition today.
- The ministry warned that patients can deteriorate suddenly. Early admission and supportive care improve the chance of survival, but they couldn't guarantee this woman's recovery.
- The eight subsequent infections include four family members and four health and care workers. All eight developed symptoms after entering quarantine. Investigators linked their exposure to contact with the first patient between October 3 and October 5.
- The ministry says that timing fits Ebola's incubation period, and investigators haven't traced an infection to transmission inside quarantine. Symptoms can appear two to 21 days after exposure. People don't transmit Ebola before symptoms begin.
- One quarantine site failed to provide a separate room, toilet and shower for each person. Officials reset monitoring dates there to October 19 as a precaution. That extended monitoring for some people beyond the first group's planned release.
- Cameras and remote discussions with doctors and community leaders were introduced to improve cooperation. The separation problem required action, even though investigators hadn't found transmission there. Monitoring people safely takes more than finding their names on a contact list.
- Authorities identified about 135 contacts. But the quarantine figures differ, with 135 in one account and 118 in another. A separate account describes a wider group of 130. Nobody has explained how those figures fit together.
- The ministry's October 22 update said all 135 contacts had been traced within Kenya and abroad. Foreign health authorities were notified. Finding someone overseas and housing someone in Kenyan quarantine are different tasks, but that alone doesn't explain the conflicting totals.
- On October 16, the contact list contained 130 people, with 127 in quarantine. By October 20, it had grown to 135. Those older figures show the list changed, but don't explain the later use of 130.
- The first release date also differs between Saturday, October 24, and Sunday, October 25. The ministry's dated plan specifies Sunday, and later reporting continues to give that date. A completed release hasn't yet been confirmed.
- The plan covers an initial eligible group. Each person's release depends on when their own last possible exposure happened.
- Contacts must complete 21 days after their last exposure without symptoms. The clinical guidance also calls for a negative test before discharge. Anyone who develops symptoms is isolated and tested. A positive result means treatment.
- Patients recovering from confirmed Ebola face stricter clearance. After symptoms resolve and treatment ends, they have a blood test following three symptom-free days. If it's negative, another follows three days later. They need two negative results. Health workers meet that standard before returning to wards.
- There is no licensed vaccine or proven specific treatment for Bundibugyo Ebola. Supportive care replaces fluids lost through vomiting and diarrhoea, treats fever and nausea, and manages complications. Clinicians have also used experimental medicines for confirmed patients.
- Those include monoclonal antibodies, lab-made proteins that bind to the virus, and the antiviral remdesivir. Their effectiveness and safety against this virus remain uncertain, with weaker evidence for remdesivir. A vaccine trial launched in Congo on October 23 is still testing possible protection.
- Ebola spreads through direct contact with a sick or deceased person's blood or other body fluids. Contaminated bedding and clothing can also transmit it. Household caregivers, health workers and people handling bodies need protection against those exposures.
- The virus isn't airborne, mosquitoes don't spread it, and living near a quarantine centre doesn't expose someone. Early symptoms can resemble malaria or other common illnesses. Fever, weakness, muscle pain, headache and sore throat can come before vomiting, diarrhoea or organ problems.
- Bleeding doesn't happen in every case and often comes later. People shouldn't wait for it before seeking assessment. Kenya's free health hotline is 719, and the Jali WhatsApp service is 0700 719 719.
- The ministry advises avoiding sick people's blood and body fluids, leaving suspected Ebola deaths to trained teams, and washing hands with soap and water. Patients should disclose travel and exposure histories promptly so staff can protect themselves while providing care.
- All 14 alert samples tested negative in the 24 hours to October 22. By then, 709,093 travellers had been screened, including 3,107 that day. Five laboratories had tested 461 samples.
- Those cumulative activities extend back to preparations for the regional outbreak in May. Screening can miss someone whose symptoms haven't started. The regional danger remains substantial. By October 20, Congo had recorded 9,704 confirmed cases and 4,687 deaths.
- An internal review at the response's start in May scored overall readiness at 66 percent. Contact tracing scored 100 percent, infection control 25, and case management 36. Points of entry scored 60 percent and logistics 49.
- Those historical scores describe preparedness then. Nearly 5,000 health workers had been trained by the October 17 account. Supplies worth about 70,000 US dollars included protective equipment, tests and infrared thermometers. More than 40 health workers had also been quarantined during the response.
- Connectivity problems affected the Malaba, Lwakhakha and Suam crossings. Reliable communications, protective equipment and trained staff are needed alongside screening. Four health and care workers became infected, but investigators haven't identified a specific lapse behind each infection.
- Kenya continues to allow travel, trade and public gatherings. The World Health Organization also advises against border closures or trade restrictions. Within Kenya, investigators still link all confirmed infections to the first patient, while monitoring continues for possible illness among contacts.
- Some contacts have lost casual income, faced anxious employers and been separated from their children. Infectious-disease specialist Loice Achieng Ombajo says people cleared from quarantine haven't had Ebola and don't have it when they leave.
- The ministry is asking employers and communities to welcome eligible contacts home. Their monitoring ends only after they meet the release requirements. Seven confirmed patients remain in treatment, and other contacts must continue monitoring until their own exposure periods are complete.
